HHIT focuses on being the safety net for the patients that are hindered by the safety net or lack thereof.  HHIT takes pride in its ability and capacity in the field of population management with great detail on the Hispanic unfunded and undocumented population within our fractured healthcare systems. Successful outcomes are obtained by HHIT’s ability to bridge discrete elements in the patient’s continuum of care process as well as applicable support aspects that endure intrinsically over time, such as patients’ values, sustained relationships, and care plans. Though other processes have been designed to improve the process of continuum of care, they do not themselves link to the success in the continuum of care process. For continuity of care to exist, the care must be experienced as connected, coherent, and culturally integrated.
